comes in. Based at Sacred
Heart Medical Center,
University District, ACE
is a program focused on
helping older adults during
and after their inpatient
treatment.
Studies conducted
during the past two
decades show that patients
in such specialized
programs are able to
return to their normal
lifestyles more quickly
than those in a traditional
hospital setting, with fewer
needing care in long-term
facilities.
At PeaceHealth, a
specially trained geriatric
care team meets each
morning to individually
review each patient’s
care plan. The team
includes a board-certifed

WHEN THE OREGON Heart & Vascular Institute (OHVI) started its
Cardiovascular Wellness & Rehabilitation program in 1986, the idea behind it—
using exercise to help heart patients feel better and live longer—was relatively
new.
Twenty-fve years and more than 10,000 patients later, the program is one of the
longest running in the nation, and its success speaks for itself in study after study.
The program offers the region’s only medically supervised exercise program in a
12,000-square-foot, state-of-the art gym—and it’s no longer just for heart patients.

provider who sees patients
only inside the hospital),
a nurse practitioner,
the patient’s primary
care provider, a physical
therapist, an occupational
therapist, a pharmacist, a
psychologist, a psychiatric
nurse practitioner, a
registered dietitian, a
chaplain, a care manager
and a social worker.
The team reviews
patients’ conditions,
problems and progress
with various assessments
and input from family
and caregivers. A patient-
friendly environment
encourages mobility
and reduces isolation to
promote faster recovery
and overall well-being.
The ACE program is
part of Sacred Heart’s
Gerontology Institute.
